A pocket scanner that works in the browser
On a phone or tablet, the tool shows a camera button. Each time you take a shot, the picture is added to the page list; you can keep photographing until the whole document is captured, or add existing pictures from your gallery or computer instead. The images are handled by the browser only, so a camera capture is not uploaded anywhere.
Photos of paper tend to look grey, uneven and shadowed. The optional document filter helps: it turns the picture to grayscale and boosts the contrast, or, in black-and-white mode, reduces it to ink on white, which is how whiteboards and printed pages usually look best. Then you order and rotate the pages, choose the page size (fit to image, A4 or US Letter), orientation and margin, and save the result as a PDF.

Taking scans that look professional
Most of the quality of a phone scan is decided before you press the button. Put the page on a plain, dark or contrasting surface, hold the phone parallel to the paper so lines do not slant, and use daylight or a lamp from the side rather than the flash, which leaves a bright glare patch on glossy paper.
Shoot one page at a time and fill the frame without cutting off the edges, since no automatic cropping is applied. Then choose the filter to suit the source: the grayscale-and-contrast mode for text with some shading or a few printed images, black-and-white for plain text pages, whiteboards and forms. If a scan looks too harsh, switch the filter off and compare.
